Case Management

Case management is a supplementary service, which involves the coordination of care among various treatment providers and/or collaboration of services among community agencies.  Some of our programs, such as DISC or Intensive Family Therapy, utilize case management more than others. 

Clients will be informed of any planned case management activities and will be asked to sign an Authorization to Release Information, allowing our staff to communicate with family, other providers or agencies, or anyone else pertinent to treatment. At the least, we try to coordinate care with any involved medical doctor or primary care physician, since our focus is on overall health and wellness.
