Integrated Care
Craig Mental Health is very excited to be involved in one of the most forward-thinking methods of providing mental health care today. Our Integrated Care program is a collaboration with the VNA/Northwest Colorado Community Health Center, and enables greater access to behavioral health care for patients who may not choose to come to the mental health center on their own. There are two behavioral healthcare professionals on site at the Health Center, providing almost 40 hours a week of availability for consults and ongoing follow-up. Because this is a collaborative program between the Health Center and Mental Health, there is an easy transition into our system for those Health Center clients who are referred to one of our psychiatrists or therapists for more intensive treatment. Referrals to the behavioral healthcare professionals at the Health Center are made by the medical staff. Usually, a patient who is being referred for a behavioral health consult will be able to meet with one of the professionals right then and there, without having to make an appointment and come back. This program has grown significantly since its inception, and we look forward to much more success as it continues to expand.
